The project develops and provides nationally consistent, practical and evidence-based clinical resources and education for health professionals. This supports proactive planning for end-of-life care, helps families and carers manage symptoms at home and allows patients to be cared for in the place of their choice.
Resources include the Prompts for End-of-Life Planning (PELP) Framework, National Core Community Palliative Care Medicines List and the palliMEDS app to support timely access to core palliative care medicines. New and updated resources are now available in the Community Palliative Care Resources Box to support personal care and symptom management for home-based patients. There are also tailored resources available to support Aboriginal and Torres Strait Islander families.
